Bleeding may exsanguinate the patient and produce shock.
The blood is unoagulable when taken in a test tube and estimation of coagulation time provides a fairly reliable side room test for the severity of en-venomation and requirement of antivenin. In survivors, renal sequelae are rare but salt losing nephritis, renal parenchymal calcification and membranous glomerulonephritis have been reported.
Death in viperine bite is due to shock, haemorrhages, secondary infection, renal failure or cardiac failure. Long term sequelae like panhypopituitarism may manifest 3-5 years after severe viperine en-venomation.
Krait venom is the most lethal on weight for weight basis. Manifestations are similar to that of a cobra bite but the local reaction at the sire of bite may be minimal. Sea snake bites are identified by the victims as sharp pricks. The local reaction may be insignificant. A Gift From A Stranger - There Can Be a Miracle When You Believe

As most of you know I have just recently underwent a Kidney Transplant. I thought I'd share a true story of mine. Something that I think you will appreciate, and believe that miracles do happen!

Throughout history the word heroism refers to people that, in the face of danger and adversity; or from a position of weakness, display courage and the will for self-sacrifice, for some greater good.
My Name is Easther Sudharta, I was diagnosed with Lupus nephritis since I was 9 years old. Lupus nephritis is a kidney disorder that is a complication of systemic lupus erythematosus. Since I was 9 I've been in and out of the hospital so many times. I sometimes feel that I am a disappointment to my parents.
My kidney problems worsen over many years, my health declined to the point that dialysis was my only option.
I began dialysis 2.5 years ago. Now, I was blessed to receive a kidney. A total stranger donated his kidney to me, liberating me after 2.5 years on dialysis.
Before The Transplant...
If you can imagine it, before this precious gift, it was becoming very challenging to do anything in my life. I was away from my full time work a lot, because of appointments. I cannot go out and stay out late with my friends because I have to dialyze in the afternoon as well as dialyse overnight, I could not travel overseas, without having to organizing my equipment to be send over.. I opted for peritoneal dialysis, which is a home therapy that offers more flexibility but is a daily process after having a emergency hemodialysis treatments. In peritoneal dialysis, a special solution is entered into the abdominal cavity and all of the toxins flow into the solution. I needed to insert 1.5 liters at a time, so it gave me a sensation of being full.
Yet my results were still not good. so they've asked me to have a fistula implanted, and do a hemodialysis instead, the word fistula wasn't in my vocabulary...I kept putting it off.
It was so exhausting for me, but I refused to let dialysis stop me from leading a normal productive life.
I was listed for a kidney transplant but the wait time here in Australia was 5 to 7 years. I thought to myself, am I going to ever receive a new kidney? But I believe that One day, that sooner or later, I will have a new kidney.
The Call From the Hospital
Long story short, On the 22 October 2011, at 10.30pm I have received a phone call from the hospital - Monash Medical Center, It was from the head of a transplant doctor, he said "Easther, I think we have found a new kidney for you."
I was stunned, I didn't know what to say, at the same time overwhelmed. Told my mom, and tears started coming down her eyes. I believe that this was a miracle. I never thought that I could receive a new kidney at all.
The joy of hearing the unbelievable good news, swiftly followed by the desire, almost panic, there was a feeling of excitement and anticipation.
The Big Day!
On October 23 2011, I underwent a surgery at Monash Medical Centre. I knew the risks, but I genuinely felt that my life was about to change dramatically as I was transported to the theater. The joy at regaining a functioning kidney is overwhelming.
The surgery was a success! I came out of the theater with a neck line a wound drip and a catheter. The catheter stays longer than what a normal transplant patient would have, as there was blood in my urine and there were clot in the bladder, so they had to do a Cystoscopy to remove the clot and healed up any scars that causes the bleeding around the bladder.
Whilst I was at the hospital, my doctor told me that my creatinine was not going down, they needed to be below 100, this show that there could be a rejection of the kidney. However they could not be sure this is what is was, so I had to have a biopsy which is where they insert a needle into the new kidney and take a bit for examination. There was risk with this, such as internal bleeding but it was the only way they could find out if the kidney was actually rejecting.
Whilst we were waiting for the result of the biopsy, the next day, doctor told me that the creatinine has gone down below 100 without them doing anything. It was such a relieve, this means the anonymous donor new kidney worked, it just needed time.
The difference in me was immediate; my color was better, my energy increased and I was eating better!
Now I am undergoing a post treatment. The doctor says everything is progressing extremely well. I have had no signs of rejection and the kidney is functioning extremely well. I visit the hospital 2x a week for a routine test.
I no longer have to do dialysis. And the only dietary restriction is grapefruit and grapefruit juice, because it affects his anti-rejection medication and everything, food wise has to be low fat, low fat milk, low fat mayonnaise etc.
The recipient was anonymous to me, all I was told was "a young man, around my age."
It is nice to know that there are such special people in this world who care about other people so much.
How do you ever find the words to express our gratitude.
I of course, had written the most heartfelt thank-you letter, which was forwarded to the donor's family. They never reply.
But I sincerely hope they realize just what it is that they've done. Out of their loss? in circumstances I don't want to imagine - came this amazing gift to me.
All the people from my childhood have always told me one thing... NEVER GIVE UP. The pursuit of happiness can be a long and rocky road, but having a family, friends, dreams and inspiration will always make me get back up no matter what. Hold your goals and your dreams dearly, never let anyone take them away!
I believe Everything happens for a reason! Nothing happens by chance or by means of good luck. Illness, injury, love, lost moments of true greatness and sheer stupidity all occur to test the limits of your soul. Without these small tests, life would be like a smoothly paved, straight, flat road to nowhere, safe and comfortable but boring and utterly pointless.

Other Health Impairment: When Does Your Child With a Medical Diagnosis Need an IEP Instead of a 504?

Other Health Impairment is a disability category under the United States federal law, The Individuals with Disabilities Education Act (IDEA). Many parents are unaware that their children with chronic or acute health problems such as asthma, attention deficit disorder, attention deficit hyperactivity disorder, diabetes, epilepsy, a heart condition, hemophilia, lead poisoning, leukemia, nephritis, rheumatic fever, sickle cell anemia, and Tourette syndrome are eligible for an Individualized Education Program (IEP). Just having one or more of these diagnoses does not make a child eligible for special education; the disability must also adversely affect your child's educational performance.
Many children have one or more of these diagnoses and function quite well at school and have no need for a specialized plan. In general, school personnel have worked hard to learn how to accommodate the needs of children with health impairments. Many children with health impairments are put on a Section 504 plan based on the Rehabilitation Act of 1973. This is not the same thing as an IEP. A 504 plan usually offers the child accommodations to help them be more successful in the general education setting. 504 plans may be adequate for many children with health impairments. However, there are three conditions that often markedly affect children's educational performance; attention deficit disorder, attention deficit hyperactivity disorder and Tourette syndrome.
Many schools will put children with these three disorders on 504 plans first, rather than assessing them for special education. If your child only needs accommodations, then a 504 plan is great way to assist your child, with a health impairment, to be more successful and to not be discriminated against. If the 504 plan is working then there is no reason to look for alternative solutions. However, if your child is on a 504 plan and their behavior is not improving and you believe their educational performance is being significantly affected by their disability you may want to request a special education evaluation.
IEPs are much more comprehensive than 504 Plans. Accommodations are only one part of an IEP. IEPs also include annual goals and objectives tied to the core content standards and specific service delivery from special education teachers and/or related services personnel (such as Occupational Therapists, Speech Therapists and Behavior Specialists). Although they are both based on United States federal laws with similar procedural safeguards, students who exhibit significant behaviors related to these disabilities are usually better served with an IEP. If your child does not qualify for an IEP, then a 504 Plan would be the next best thing. If you decide to request a special education evaluation, put it in writing. Each state has laws in effect pertaining to how long after the school district receives the request that they need to meet with you to have you sign permission for an evaluation to begin.

Kidney Failure in Dogs - Causes, Symptoms and Treatments

All of our bodies suffer from the wear and tear of time. In people, hearts are often the most vulnerable. In dogs, it is often the kidneys that wear out first. There is a slow, but steady and gradual loss of kidney function in all of our pets as they age. The fact is that nine in every 1000 dogs that are examined suffer from chronic renal disease, and most severely, chronic renal (or kidney) failure is the leading cause of illness in older dogs.
Kidney failure is defined as the kidney's incapacity of removing waste products from the blood. The accumulation of toxins produces the signs and symptoms of uremic poisoning. Kidney failure can appear suddenly (acute kidney failure) or happen gradually over months (chronic kidney failure). Most cases are of the gradual onset type and are caused by nephritis.
First of all, there is something we should know about what role kidney plays in maintaining the healthy life of your dog. Kidney - among with other things regulates blood sugar, blood pressure, blood volume, water composition in the blood, and pH levels, and produces red blood cells and certain hormones. Kidney failure can take place so slowly, and more severely, often we find out the disease when the symptoms are so clear, which means it may be too late to cure it effectively. In fact, dogs with kidney failure do not show signs of uremia until 75 percent of functioning kidney tissue is destroyed. Thus, a substantial amount of damage occurs before the signs are noticed.
What are the signs of kidney disease in dogs? One of the first things you may notice is that your dog drinks water and urinates more than usual. At first, this could be so normal that most owners ignore it. It might just be your dog wakes you up many times during the night to let her out or you need to fill the water bowl more than you used to before. These signs are due to the failure of kidneys to concentrate the urine. This leads to a large urine output over which the dogs are unable to control, with subsequent dehydration and thirst.
As kidney function declines, the dog retains ammonia, nitrogen, acids, and other chemical wastes in her blood and tissues. This stage is called uremia. The degree of uremia is determined by measuring serum blood urea nitrogen (BUN), creatinine, and electrolytes. Signs of uremia are apathy and dression, loss of appetite and weight. You will see your dog lose weight and become a more finicky eater. Dogs at this stage may urinate less than normal. Ulcers may arise in mouth. With the nephritis syndrome the dog develops edema and ascites. Vomiting, gastrointestinal and diarrhea, bleeding may occur. At the end stages of kidney failure, the dog falls into a coma.
As mentioned earlier, symptoms often occur gradually for a long time. Actually, dogs with kidney failure do not show signs of uremia until 75 percent of functioning kidney tissue is destroyed. Thus, a considerate amount of damage occurs before signs are noticed.
Kidney failure can be resulted from many reasons:
- Kidney diseases
- Poisoning, especially from antifreeze
- Complete urinary tract obstruction caused by a stone
- Congestive heart failure with low blood pressure and reduced blood flow to the kidneys
- Rupture of the bladder or urethra
- Shock, with inadequate blood flow to the kidneys
- Lyme disease
- Leptospirosis
Treating kidney failure
While chronic kidney failure cannot be reversed or cured, treatment and management aimed at reducing the contributing factors and symptoms can slow its progression.
Treatment of kidney failure in dogs takes place in two phases.
The first phase is to "restart" the kidneys. Large quantities of intravenous fluids are given to clean out the kidneys. This cleaning process, called diuresis, helps to get the kidney back to function again. If the dogs retain enough functional kidney cells, body's needs for waste removal maybe met. Fluid therapy includes replacement of various electrolytes, especially potassium. Other important things of initial treatment include proper nutrition and drugs to control vomiting and diarrhea.
The three possible outcomes from the first phase of treatment of kidney failure in dogs:
1) After resuming the function during the treatment, the kidney continues to work well for a few months or years;
2) During treatment, the kidney works well, but stops functioning as soon as treatment stops;
3) Kidney function will fail regardless of treatment. Unfortunately, there is no reliable proof to know what outcome will fall in.
The second phase of treatment in dogs is to keep the kidneys functioning as long as possible. This shall be achieved by one or more of the following treatments, depending on the situation:
1. A special diet. The ideal diet is not acidifies, low in protein and phosphorus. This kind of diet helps to keep the blood tests as close to normal as possible, which often makes your dog feel better. Also, once kidney disease is becoming severe, a decreased protein diet will decrease the workload on the kidneys, thus helps the dogs suffer less the kidney failure.
2. A phosphate binder. Phosphorous will be removed from the body by filtering through the kidneys. Once the filtration process is impaired, phosphorous begins to accumulate in the blood. This also contributes to lethargy and poor appetite. Certain drugs will bind excess phosphates in the intestinal tract so they are not absorbed, resulting in lower blood levels of phosphorus.
3. Fluids given at home. When your dog is stabilized, fluids can be given under the skin. This serves to continually "restart" the kidneys as their function begins to fail again. Depending on the degree of kidney failure, this treatment can be done once daily to once weekly.
4. A drug to regulate the calcium and parathyroid gland levels. The calcium and phosphorus must remain at about a 2:1 ratio in the blood. As mentioned above, the increase in blood phosphorus level stimulates the parathyroid gland to increase the blood calcium level by removing it from bones. This can be helpful in order to normalize calcium: phosphorus ratio, but it can make the bones brittle and easily broken. Calcitriol could be used to reduce the function of the parathyroid gland and to increase calcium absorption from the intestinal tract. This is recommended if there is evidence of abnormal function of the parathyroid gland.
5. A drug to stimulate the bone marrow to produce new red blood cells. The kidneys produce erythropoietin, a hormone that stimulates the bone marrow to make red blood cells. Therefore, many dogs suffer from kidney failure have a low red blood cell count, anemia. Epogen (or Procrit), synthetic forms of erythropoietin, will correct the anemia in most dogs. However, for some dogs, this drug cannot be used long term since the immune system recognizes the drug as "foreign" and will create antibodies (immune proteins) to against it. This is recommended if there is persistent anemia present.

Diseases of the Kidneys

Kidney are bean-shaped organs, lying below the waist on either side of the vertebrae, are only about four inches long, two inches wide, and an inch thick. Yet arranged in orderly precision around the outer portion are millions of tiny balls of capillaries, or glomeruli-the Latin plural for ball. Long, slender tubes, or tubules, curve around each miniature ball, then after winding like hairpin loops on a mountain road, converge into ever larger tubules until all combine into a single canal, the ureter, leading to the bladder. Capillaries carrying fresh blood surround each tubule; and the whole is held together by connective tissue. Thus incredible miles of capillaries and tubes form the working mechanism of the kidneys.
An adult has about 12 quarts of blood, yet because of continuous circulation, the kidneys filter approximately 4,000 quarts daily. Elevated blood pressure in the capillary balls forces protein-free plasma into the tiny tubules; and as this plasma trickles down its long path, water and nutrients are reabsorbed into surrounding capillaries much as foods are absorbed from the digestive tract. In this manner, healthy kidneys prevent dehydration, throw off wastes (largely urea from the breakdown of worn-out cells), and keep the body from becoming acid or alkaline.
Infections of the kidneys and bladder. As with other infections, inflammations of the kidneys and urinary bladder, known as pyelitis and cystitis respectively, call for an anti-stress diet and increased amounts of all nutrients needed to build resistance. Because a highly acid urine, produced by eating meats, eggs, milk, cheese, breads, and cereals, retards bacterial growth, vegetables, fruits, and especially citrus juices should be largely avoided for a week or more.
During cystitis, ulcers sometimes cover the walls of the bladder, and unless vitamin E is kept unusually adequate, massive scarring can form, contract, and drastically reduce its capacity for storing urine.
Problems With Voiding
Inability to control the passing of urine, or "bed-wetting," has been produced in volunteers by a deficiency of magnesium. Conversely, inability to pass urine occurs when the blood potassium becomes low following severe stress; and it has been produced in animals by diets deficient in vitamin B2 or pantothenic acid. Supplying the missing nutrients rectifies the situation.
Bright's Disease
The term Bright's disease is applied to many types of kidney damage, including nephritis, an acute inflammation most often affecting children but usually not involving bacteria. Blood albumin, antibodies (gamma globulins), and millions of dead cells are lost in the urine, and the blood serum is often milky with fat and cholesterol. This disease can become progressively worse and result in death, or it may subside into a chronic stage where the patient gets better but not often well.
Nephrosis refers to less acute kidney damage generally affecting older people. Its onset is usually gradual, though puffiness under the eyes, frequent urination, perhaps headaches and excessive blood cholesterol may precede it for years. Fatty deposits in the arteries and tiny tubules decrease the blood supply to the kidneys, the amount of nutrients reabsorbed, and the quantity of urine excreted. Entire areas may be filled with dead cells and scar tissue, yet recovery is still possible. Usually when urine formation is severely inhibited, highly toxic urea cannot be excreted and may cause fatal uremia poisoning.
Numerous other names given to kidney disease refer to the part of the organ most damaged and are transitional phases of the same abnormality.
Experimentally Produced Kidney Diseases
Nephritis has been produced in many species of animals by diets deficient in the B vitamin cholin. The coils of capillaries are particularly damaged, severe hemorrhages occur, blood lecithin drops far below normal, deposits of cholesterol and fats become so excessive that urine formation is decreased, circulation inhibited, and so much albumin is lost in the urine that dropsy results. Vitamin B12, folic acid, or the amino acid methionine alone can correct the dropsy, but only cholin can stop the hemorrhaging.
When diets high in calories, especially alcohol or refined sugar, are given, the need for cholin is markedly increased and kidney damage becomes much more severe. If the diet is low in both cholin and protein, such acute dropsy develops that digestion, circulation, and other body functions are interfered with. Although cholin deficiency also harms the liver, kidney disease is produced long before liver damage can be detected.
Cholin can be made from methionine, but when growth is rapid, the building of body protein has priority and leaves none of the amino acid to be changed into the vitamin. Giving cholin with inositol or as lecithin, however, is much more effective than cholin alone.
Though the experimentally produced nephritis, "strikingly similar to the disease seen in man," is corrected as soon as cholin is added to the diet, this vitamin is rarely given to humans with kidney diseases. Yet more than half of 102 people with fatty livers, recognized as a cholin deficiency, had high blood urea and albumin in the urine, showing mild nephrosis, which quickly disappeared when cholin was given with an adequate diet. In another study of 48 persons, the blood pressure fell to normal and albumin cleared from the urine when cholin was taken.
